| Information
Sevoflurane (Sevo) is the most recently introduced and safest inhalant anesthetic available in veterinary medicine. Sevo provides for a more rapid, and thereby smoother anesthetic induction as well as a smoother recovery period. Sevo also allows for very precise control and adjustment of anesthetic depth. Sevo has minimal effect on the heart and respiration and is highly recommended for senior pets (those over seven (7) years of age) and those pets with known medical health concerns.
Isoflurane (Iso) is the most widely used inhalant anesthetic in veterinary medicine. Isoflurane is a very safe anesthetic. Iso is the inhalant anesthetic used at Parkway Animal Hospital for more than eight (8) years. Iso, however, does not have the properties allowing for as rapid and smooth an induction and recovery as Sevo does. Both Iso and Sevo are extremely safe inhalant anesthetics. The average healthy pet will do fine with either anesthetic, but the side effects, induction and recovery are easier on your pet using Sevo. For this reason, Sevo is the preferred anesthetic by the doctors at Parkway Animal Hospital.
Parkway Animal Hospital is sensitive to the impact of anesthetic price in the overall cost of elective procedures. The cost of using Sevoflurane for most procedures is an additional $25-$30 over Isoflurane. For this reason, we are leaving the decision up to you, the informed client, to choose the inhalant anesthetic used for your pet's procedure. In certain instances (pets with known medical concerns), Sevo WILL be the anesthetic used. This decision will be made at the doctor's medical discretion.

I have read and understand that I have the opportunity to choose the inhalant anesthetic used for my pet's procedure. I understand that while both anesthetics are very safe, one anesthetic (Sevoflurane) has distinct advantages over the other anesthetic available (Isoflurane). I also understand that the use of Sevoflurane will incur an additional fee of $25-$30 for the average procedure.
Based on the information given, I select the use of: (please initial below)
